Professor Marshall’s groundbreaking discovery of the link between Helicobacter pylori, gastric diseases, and gastric cancer reshaped the global management of digestive diseases, set a milestone for early prevention and treatment of gastric cancer, and provided strategic guidance for Xiamen’s regional early cancer control efforts.
项目背景与必要性 / Background and Rationale
当前,胃癌和结直肠癌仍是厦门市高发的重大慢性疾病,消化道早期癌症治愈率可超过90%,而晚期不足30%。
At present, gastric cancer and colorectal cancer remain major chronic diseases with high incidence in Xiamen; cure rates for early‑stage gastrointestinal cancers can exceed 90%, while those for late‑stage disease are below 30%.
实践充分证明,“早筛、早诊、早治”是降低癌症死亡率、节约医保基金最经济、最高效的路径。
Practice has fully demonstrated that “early screening, early diagnosis, and early treatment” is the most cost‑effective way to reduce cancer mortality and save medical insurance resources.
立足“健康中国”战略部署,紧扣厦门市智慧卫健、智慧医保建设布局,项目依托厦门大学医学中心学科与平台优势,由厦门大学附属翔安医院在马歇尔教授指导下牵头实施“翔安区、思明区幽门螺杆菌+消化道早癌一体化社区筛查闭环项目”。
In line with the Healthy China strategy and Xiamen’s plans for smart healthcare and smart medical insurance, the program leverages the disciplinary and platform strengths of Xiamen University Medical Center and is led by Xiamen University Affiliated Xiang’an Hospital under the guidance of Professor Marshall to establish an integrated community screening and management project for Helicobacter pylori and early gastrointestinal cancer in Xiang’an and Siming Districts.
项目总体布局 / Overall Program Design
项目立足翔安、思明两大主城区,辐射闽西南地区,构建“科研示范+规模推广”的双轮驱动发展格局。
The program is based in Xiang’an and Siming, the two main urban districts, and extends to southwestern Fujian, forming a dual‑engine framework of “scientific demonstration and large‑scale promotion”.
以厦门大学附属翔安医院为龙头,联动思明院区及辖区各社区卫生服务中心,整合厦门市抗癌协会、香港理工大学、芨影科技等多方资源,搭建“社区宣教、AI初筛、保险兜底、内镜精诊、规范根除、基层随访”的全流程闭环管理体系。
Taking Xiang’an Hospital as the lead, the program links the Siming campus and community health service centers, integrating resources from Xiamen Anti‑Cancer Association, The Hong Kong Polytechnic University, and Jiying Technology to build a full‑cycle closed‑loop management system covering community education, AI‑based preliminary screening, insurance coverage, endoscopic diagnosis, standardized eradication, and primary‑care follow‑up.

It aims to create a prevention and control model in Xiamen that can be replicated, scaled up, and extended to the wider southwestern Fujian region.
目标人群与技术路径 / Target Population and Technology Pathway
未来3至5年,项目将实施分层精准筛查,聚焦35至70岁常住人群,重点覆盖40至60岁中坚群体。
Over the next three to five years, the program will implement stratified and precise screening, focusing on permanent residents aged 35 to 70, with particular attention to the 40 to 60 age group.
对具有癌症家族史、消化道症状、长期烟酒及高盐饮食等高危人群进行重点建档和分级管理。
High‑risk groups with a family history of cancer, gastrointestinal symptoms, or long‑term smoking, alcohol consumption and high‑salt diets will be prioritized for registration and tiered management.
项目将创新智能技术路径,运用大语言模型初筛系统开展低成本、无创风险分流,精准锁定高危人群优先安排内镜检查,科学调配医疗资源,避免内镜挤兑,提升筛查效能。
The program will adopt innovative intelligent technologies, using a large‑language‑model‑based triage system to provide low‑cost, non‑invasive risk stratification, accurately identifying high‑risk individuals for prioritized endoscopic examination, optimizing resource allocation, preventing endoscopy overload and improving screening efficiency.
支付保障与服务模式 / Financing and Service Model
在支付端,项目将构建多元支付保障体系,建立“医保统筹+惠厦保商业补充+市区财政补贴”的三元支撑机制,切实降低群众筛查与治疗成本,提升全民主动参与意愿。
On the financing side, a diversified payment mechanism will be established, combining basic medical insurance pooling, the Huixia supplementary commercial insurance and municipal‑district fiscal subsidies to reduce screening and treatment costs and enhance public participation.
在服务模式上,项目将健全三级分诊机制,固化“首诊在社区、诊治在专科、慢病随访回基层”的服务流程,持续强化基层家庭医生服务能力,筑牢基层防控网底。
In terms of service delivery, the program will refine the three‑tier referral system, institutionalizing the model of “first contact at community level, treatment in specialist centers, and chronic disease follow‑up back in primary care”, thereby strengthening family doctor services and reinforcing the grassroots prevention and control network.
预期成果与长期愿景 / Expected Outcomes and Long‑Term Vision

项目计划在全市完成累计不少于200万人次的幽门螺杆菌筛查,规范根除治疗率稳定达到80%以上。
The program plans to conduct screening for Helicobacter pylori in no fewer than two million person‑times citywide, with a standardized eradication treatment rate consistently above 80%.
同时,将同步建设“闽西南幽门螺杆菌与消化道早癌大数据中心”,构建风险预测模型和居民专属消化道健康档案,为市级医保优化和公共卫生决策提供真实世界数据支撑。
In parallel, a “Southwestern Fujian H. pylori and Early Gastrointestinal Cancer Big Data Center” will be established to build risk prediction models and individual gastrointestinal health records, providing real‑world evidence for optimizing municipal medical insurance and public health decision‑making.
从长期看,通过持续筛查与干预,力争在10至15年内实现区域胃癌发病率和死亡率稳步下降,从源头压降晚期癌症救治费用,真正惠及民生,减轻医保负担。
In the long term, through continuous screening and intervention over 10 to 15 years, the program aims to achieve a steady decline in regional gastric cancer incidence and mortality, reducing late‑stage treatment costs at the source, improving public health and alleviating pressure on medical insurance.

项目在中共厦门市委、市政府和市委组织部的指导下推进,得到了市卫健委、市科技局、市科协等部门的大力支持。
Implemented under the guidance of the CPC Xiamen Municipal Committee, the Xiamen Municipal Government and the Organization Department, the program has received strong support from the Municipal Health Commission, the Science and Technology Bureau and the Association for Science and Technology.

Professor Marshall’s team, represented by Jiying Technology, is deeply involved in the top‑level design of the project, contributing to Xiamen’s H. pylori control strategy and the development of community screening standards and technical specifications.
项目还将积极探索设立国际人才交流基地,打造厦门全球医学人才合作新窗口;依托厦门大学,进一步深化与西澳大学、香港理工大学的学术联动,共建科研平台、联合申报课题、定向培养专业人才,并向泉州、漳州、龙岩等地输出技术标准与内镜人才培训体系,构建闽西南消化道防控一体化格局。
The project will actively explore the establishment of an international talent exchange base to create a new window for global medical collaboration in Xiamen; leveraging Xiamen University, it will deepen academic cooperation with the University of Western Australia and The Hong Kong Polytechnic University, co‑build research platforms, undertake joint projects, train specialized professionals, and export technical standards and endoscopy training systems to Quanzhou, Zhangzhou and Longyan, fostering an integrated gastrointestinal disease control network across southwestern Fujian |
